Uhm.. "every single fic"? Excuse me, but that is a bit inaccurate. The
following are the "Ranko fics" that I can remember off-hand (if I'm
forgetting anyone's fanfic please feel free to correct me.) I'm
including those that have Ranma's male and female sides as different
personalities.

Split Personalities
Careful Destiny
Nullifier 122
Hemispheres
Twilight Existence
Curse of Darkness
Onna: Red Side
Song of the Phoenix <-- Shameless plug ;P

Among all of these, only Split Personalities blatantly pairs up Ranma
and Ranko, however it's done only for the lemon scenes. Careful Destiny
has Ranko herself thinking about her brother that way, and Nullifier
*forces* them to lust for each other. None of the others even *hint* of
doing something similar, though.

Let's look at the thought process:

They act the same

Split Personalities: Ranko is more "extroverted" and knows more about
the world than Ranma.

Careful Destiny: Ranma is the same as always, but Ranko acts like a
(somewhat) normal girl, if a bit hot-tempered.

Nullifier: Since Ranma-chan was stuck as a girl, she seems to do much
more thinking than the male Ranma, who is still quite reckless anyway.

Hemispheres: Bad Ranma, Good Ranma: obviously different; onna-Ranma is
the same, though.

Twilight Existence: Ranko doesn't even share Ranma's taste for martial
arts.

Curse of Darkness: Senshi *wants* to kill Akane. How's that for acting
the same?

Onna: Red Side: Well, if Hikaru-chan acts the same way as Ranma then
Rumiko Takahashi should sue CLAMP...

Song of the Phoenix: Ranko is *much* more mature than Ranma. She isn't
reckless anymore, and thinks before speaking. She's not even arrogant
anymore.

Share the same facial features

True for all.

Share the same hairstyle

True, although most have Ranko letting her hair down eventually.

both share the same sport/occupation

Twilight Existence's Ranko doesn't. And in Onna: Red Side, Hikaru's
interested in *kendou*.

etc. etc.

Wow, they must be romantically involved.

Wrong.   Wouldn't a normal person assume that THEY ARE RELATED maybe even
TWINS?

Split Personalities: everyone thinks Ranko is Ranma's "cousin," and
Gosunkugi and Tsubasa find out they were one and the same.

Careful Destiny: they *are* siblings.

Nullifier: unclear about the subject.

Hemispheres: Ranma was back to normal before any strange gossip started.

Twilight Existence: a bit unclear.

Curse of Darkness: I'm figuring Ranma would kill anyone stupid enough to
say that he and Senshi have something going ^_^

Song of the Phoenix: again, siblings. And everyone knows it.
 

As you can see, even in the fanfics where Ranma and Ranko *are* paired
up (sexually or romantically)  no one "thinks" that they are emotionally
related. In fact, I can't think of *any* fanfic where people see them
that way. However, the only places where a otoko-Ranma/onna-Ranma
relatinship has been suggested are: Kodachi's appearance in the MANGA
(where SHE believed Ranma-chan was out to conquer "Ranma-sama"), and the
MANGA volume #35, in which the mirror-copy actively pursues Ranma.

If you can produce any evidence of other fanfics in which such a
relationship is hinted by *other* characters, please do so.

And to reply to your other question: to why nobody figures out why Ranma
and Ranma-chan are named the same way:

Split Personalities: different names right from the start. Ranma-Ranko.

Careful Destiny: ditto.

Nullifier: well... it's a bit iffy. Time will tell when more chapters
are written.

Hemispheres: it took place in *one* night. Not enough time for anyone
else to wonder why they had the same name. OTOH, the three beings *were*
different sides of Ranma's psyche.

Twilight Existence: Ranma-Ranko again.

Curse of Darkness: Ranma-Senshi.

Onna: Red Side: Ranma Saotome-Hikaru Shidou.

Song of the Phoenix: Ranma-Ranko/Zhaodi. She even gets a third name much
later in the story.
