Well as far as Ms Tendo's name, I don't think it was ever mentioned.  
But I may be wrong.

As far as how to address people with respect(as if any one other than 
Kasumi ever bothers), I am no expert, but I can help a little.

Tofu and any of the teachers, or Mr. Tendo, if he bothers to teach, 
would be addressed at -sensei.  Example: Tofu-sensei.  It means 
master or teacher.

Any of the mail characters can be addressed as -kun.  This is a term 
of familiarity, used when referring to a male individual.  Example: 
Ranma-kun or Ryouga-kun.

-san would be used to refer to a stranger or new aquantance.

-sama, would be used to refer to an elder, or the head of household, 
or someone of high social class.

-chan is the diminutive.  Little.  Usually used in reference to a 
small child, or young girl.  Also occasionally used by a girls 
boyfriend, or occasionally a boys girlfriend, or a persons parent.
