and I am passing along the advice to you. In other words Repace all question
marks and periods in your dialogue with commas.

 ' "How are we feeling?" he asked. '  is a perfectly correct
construction. You should ALWAYS punctuate dialog as it is pronounced,
the only unintuitive trick is that descriptive tags are separated by
commas with the period following. Example:

"He gave me an example to follow," said the commentator.

See also:

"He gave you an example?" the student asked the revered fanfic writer.
"He gave you an example!" the student concluded triumphantly.

Example #2

    "I'm sorry," The man apologised "It's simply that we have some questions

No, this is wrong too. The punctuation that falls within the quotes does
not end the sentence even if it's a question mark or exclamation point,
DEFINITELY not just from a comma.  ' "I'm sorry," he said. ' would be
correct.

I'd also recommend snipping more of the long passages when commenting
like you do; leaving a paragraph before the comments is a good amount
for the author to find his place in, and then you can easily put a few
more blank lines in to separate the commented bits.
